The tournament team check-in, which EVERY TEAM MUST ATTEND, will be held Friday August 6th, 6-8:30 PM at the Hembree Park Recreation Building, 850 Hembree Road Roswell, GA 30076.
At Check-in, you will need the following items:
Team Tournament Liability Waiver Form (Found under Documents above)
A player card for each player (Fall 2010-Spring 2011 or Fall 2009-Spring 2010)
A Medical Release Form for each player
